Medical Care Manager position at the Bronx HCC:

Responsibilities 
The Medical Care Manager (MCM) blends the mission of AHF with Medical Care Management, utilizing their experience, skills, and education with a thorough understanding of the Retention & Adherence Program (RAP) within their specific area.

With a caring and knowledgeable focus, you will work with our providers and healthcare center team so that every eligible client receives the support they require and access to available resources. This will include providing education and assistance so that eligibility for the program is understood. Working directly within the Healthcare Center, you will meet with patients and complete Health Risk/Needs Assessments, develop Service/Action/Care Plans, and provide ongoing education to clients so that they understand their diagnoses and their medications.

For RAP contracts that combine psychosocial needs within Medical Care Management, you will understand mental health needs and basic needs such as housing, food, and clothing and develop relationships with programs and agencies serving those needs and facilitate referrals, support, and assistance for your patients who lack those services.
 
By utilizing your care management experience, you will meet our clients where they are, identify strengths and opportunities for growth and assist them with realistic goal setting. With your skills of collaboration and engagement, you will bring the essence of the safety net for the uninsured HIV/AIDS clients - to our most vulnerable population.

By establishing Health Risk/Needs Assessments, Action/Service/Care Plans, referrals and support, your work will directly contribute to AHF’s success in meeting the deliverables of our Retention & Adherence contract. Beyond the scope of the contract, your work contributes to measures established by the HIV/AIDS Bureau (HAB) Measures and the Department of Health and Human Services.  
 
The MCM is often the liaison between provider and client and is able to facilitate the shift from leaving the exam room to implementing the optimum plan of care and putting it into action. Providing brief interventions focused on a long-term achievable goal, the client and the MCM work together each step of the way towards viral suppression, better health outcomes, and quality of life.

Education and/or Experience
•    Medical Care manager must meet one of the minimum qualifications: 
          a. A bachelor’s or master’s degree in social science.
          b. A bachelor’s degree not in social science with at least 6 months of case management experience.           
•    At least three years of social work experience in a medical environment is preferred.
•       Knowledge of community resources
•       Knowledge of the target population
•       Knowledge of HIV disease and treatment
•      Competence in Cultural and linguistic Diversity.

Job Details:
The Peer Coordinator will be responsible for providing client centered support and education to clients aging with HIV. Through a collaborative effort with the client’s care team, the Peer will coordinate and advocate for services that will promote positive health outcomes. Peers have a deep understanding of what the patient is experiencing; Patients have an opportunity to discuss personal issues with Peers in a safe environment and with someone who can relate to their situation.

Responsibilities:
•    Identifies and addresses barriers to care, anticipate and pre-empt potential problems, promote patient self-efficacy through teaching and coaching; and provide emotional support – HIV positive with success in managing their HIV care and treatment as well as navigating health care systems
•    Responsible for the orientation of new patients to grant support services and to clinic services. In addition, introduce new patients to clinic staff
•    Navigation of patients to medical visits or other health and human services appointments.
•    The Coordinator is expected to outreach all new referrals within 5 work days, make reminder calls and follow-up for missed appointments.
•    Provide educational and organizational materials
•    Support education and adherence assistance through individual or group activities.

•    Ensure that program goals of client outcomes are documented in AIRS so that annual service units can be met and demonstrate ability to set priorities and manage time in an efficient manner.
•    Responsible for documenting all interventions conducted with the client or on their behalf in AIRS in a manner that is accurate and error free. Track all interventions on the program specific client contact logs. Documentation is to be completed immediately following the intervention and program specific monthly monitoring data is to be submitted no later then the 5th of the following month.
•    Will attend the AIDS Institute Peer Certification program to completion
•    Ensure that client’s confidential health care and personal information is protected at all times. Responsible for ensuring that appropriate consent forms are signed and updated as required. Ensure that exchanges of information are only disclosed to those individuals for which client consent has been obtained. Responsible for the safeguarding of electronic devices and passwords.
 •    Work as a team player in all areas to enhance the program as a whole. Hear staff and client concerns in an open and solution-oriented manner and refer all case management/client needs to the Administrator ACC.
 •    Responsible for actively participating in the Department’s Quality Improvement and Quality Assurance efforts, including but not limited to participation in the Department’s Quality Improvement Committee.

Qualifications/Experience Requirements:
•    Living with HIV with proficiency in managing care and medication adherence
•    Experience with sexual health education, harm reduction strategies preferred
•    Comfortable with communicating with patients and within the community

Education:

Licenses / Certifications:

  • Accurately complete all types of account transactions requested by members by following organizational processes and procedures.  This includes processing negotiable instruments, accurate end-of-day proof and handling large volumes of cash.
  • Open new memberships, new accounts, and new loans and process loan and account transactions for retail and business members. Use needs based sales approach, ask questions to uncover ways to meet members’ financial needs with additional HVCU products and services.

Rainbow Heights Club is seeking a part-time, virtual training co-facilitator to support the enhancement and expansion of the E/Quality Care training program. The E/Quality Care Training Co-Facilitator will be responsible for collaborating directly with Rainbow Heights Club staff to prepare and co-facilitate LGBTQ+ and TGNC cultural competency trainings for health and human services providers in New York State.

Co-Facilitator Responsibilities:

  • Co-facilitate between 3-5x E/Quality Care training webinars per month via Zoom with Director of Community Engagement
  • All training scheduling with external agencies will be handled by Director of Community Engagement; Co-Facilitator is expected to respond in a timely fashion to availability queries
  • Onboarding will include paid time spent viewing, evaluating and providing feedback on content and presentation of LGBTQ+ and TGNC trainings, to ensure their alignment with co-facilitator’s own clinical expertise and lived experience

Co-Facilitator Qualifications:

  • Education: Master’s degree and licensure in Social Work or a related field
  • Clinical Experience: A minimum of 2 years of clinical experience in the provision of behavioral health services in a community setting. Clinical experience working with LGBTQ+ (particularly TGNC) adolescents a huge plus, but not required.
  • Training Experience: A verifiable track record of developing and delivering cultural competency or continuing education trainings for health and human services providers
  • Communication Skills: Effective oral communication and presentation skills – particularly, comfortarticulating the clinical benefits of LGBTQ-affirming care, as well as answering participant questions and providing practice suggestions in real time

Location: Applicants must live and be familiar with the service landscape in New York State, but the duties of the position can be fulfilled virtually.

Hours/Duration: 8-15 hours per month. Trainings typically take place on Tuesdays before noon, but this position will periodically require scheduling flexibility beyond that. The training contract runs through the end of June 2023, at which point there could be the opportunity to renew for the following fiscal year.

Compensation: Training co-facilitator will be paid as a consultant at an hourly rate to be negotiated based on experience.

Community Engagement Specialist

Responsibilities:

  • Responsible for the oversight of YTG persons of color (POC) employment program comprised of three YTG persons of color. This includes recruitment, providing weekly supervision, professional development, ongoing feedback and skill assessment, and maintaining employee records.
  • Establish and maintain YTG POC community advisory board which will meet once per week. Collaborative activities must focus on the development and execution of events, regional Discord channel, and addressing social determinants of health.
  • Conduct HIV/STI/PrEP/PEP testing and screening services within an HIV status-neutral model of care. This includes conducting behavioral risk screenings, social determinants of health screening, providing personalized cognitive counseling, and linking clients into case management to promote ongoing care engagement regardless of HIV status.
  • Assist with the identification of key YTG POC influencers to promote testing services within their social networks
  • Identify new medical and essential support service providers who have a history of proving services to YTG community.  This includes attending initial meetings, assisting with the formalization of working agreements, and tracking agreement renewals.
  • Develop and execute new internal date-to-care program in collaboration with data team and PFY Director with the goal of re-engaging out-of-care persons living with HIV/AIDS into care.

HIV Services Navigator II

Full-time:  37.5 hours/week                                                      

Reports to: Prevention Services Coordinator

Responsibilities:

  • Provide community and social media-based outreach to young transgender YTG) persons of color  (ages 13-34) throughout Queens, Nassau, and Suffolk Counties. Outreach will focus on engaging YTG persons of color in HIV/STI testing, Pre-Exposure Prophylaxis (PrEP) and Post-Exposure Prophylaxis (PEP) services. Community outreach venues will include local bars, nightclubs, street/neighborhood locations where YTG persons of color gather. Social media outreach will include Facebook, Instagram, TikTok and dating apps such as Grindr. Outreach will include giving out safer sex packets, providing education on PEP/PrEP, and the director provision of HIV/STI testing services.
  • Conduct HIV/STI/PrEP/PEP testing and screening services within an HIV status-neutral model of care. This includes conducting behavioral risk screenings, social determinants of health screening, providing personalized cognitive counseling, and linking clients into case management to promote ongoing care engagement regardless of HIV status.
  • Provide confidential HIV/STI testing and counseling for at-risk individuals during event programming, via mobile testing, and by appointment. Must maintain all HIV testing logs, equipment and records.
  • Coordinate, promote, and ensure linkage to ongoing medical services for clients demonstrating need for anti-retroviral medication, PrEP, or PEP.
  • Provide support and assistance to link all clients who are newly diagnosed with HIV to Partner Notification services.
  • Provide linkage support to all testing clients to promote engagement in essential support services (ex: food pantry, housing assistance, insurance navigation, name change)
